Attendance Reporting

Attending school every day has a positive impact on student learning and helps to put students on a track towards success. Parents and guardians are responsible for ensuring that children attend school daily and must authorize any absences.
Excused absences may include illness, observance of a religious holiday, death in the immediate family, family emergency, and situations beyond the control of the student or family. Students who are absent for more than three consecutive days due to illness may require a physician's note upon return.
Reporting Procedures

Daily Absences
Call your child's school to report by 10 a.m.Provide name of the child, grade, homeroom teacher, and reason for the absence. If no contact is made within one day of the absence, it will be considered unexcused.
Note: As a safety precaution, parents/guardians must call the school directly to report the absence.

Longterm Absences
If an absence lasts more than a week, the student or parent/guardian are encouraged to make arrangements with the classroom teacher to arrange for completion of missed classwork. Please note teachers are only required to provide assignments during excused absences and suspensions.

Mid-day Absences
Students may leave during the school day only when authorized by the principal after receipt of a written note requesting permission from a parent/guardian , and when the student is picked up from the school office.

Ensure parent/guardian and emergency contact information is kept up-to-date in the event of an illness at school or unforeseen circumstances. Information can be updated directly in the PowerSchool Parent Portal, by calling the school office, or sending an email to registrationinfo@district65.net.
Other Attendance Information

If a physician anticipates that a student will miss 10 days of school or that the student has a chronic medical condition that will cause the student to have sporadic attendance, home hospital tutoring may be provided. Parents/guardians must obtain a written request from the family physician for these services. Tutoring by a certified teacher for approximately five hours per week can be provided. The tutor will collaborate with the child’s teacher(s) to ensure appropriate instruction and assignments are provided during this time.

Planned Absence Form (3-9 School Days)
If a family has a planned absence, the parent(s)/guardian(s) are responsible for submitting this information in writing to their school's administrative assistant or nurse no less than two weeks before the student's absence. The Planned Absence does not excuse a student's absence.
Planned Absence Form (10 or More School Days)
If a family has a planned absence for 10 or more consecutive days, the parent(s)/guardian(s) are responsible for submitting this information in writing to their school. Please contact your school administrative assistant or nurse to notify them and complete the form.
Students will NO LONGER BE WITHDRAWN for absences of more than 10 days. The state determines if absences are excused or unexcused, and vacations are unexcused.

Continuous daily attendance is necessary for overall student success in school. We are committed to all of our students having a 90% or higher attendance rate. Parents/guardians are expected to stay in communication when a student is out for any reason. When attendance issues arise, interventions will take place to support improving student attendance.
In the State of Illinois, when a student has (9) or more unexcused absences, the student is considered chronically truant.
When a student has (18) or more absences, excused or unexcused, the student is considered chronically absent.
A member of our team will communicate with parents/guardians regarding their child’s absences via emailed attendance notices during the school year. These notices are sent on the 5th and 9th days of unexcused absences and the 10th, 15th and 18th days of overall absences.
If a student’s attendance does not improve after notices have been sent, the following interventions may take place; a follow-up phone call, a parent/staff meeting held at school, a referral to the truancy intervention program with a meeting held at the Joseph E. Hill Administration Center, or a referral to the North Cook Regional Office of Education Truancy Officer.

In accordance with Illinois state attendance codes, vacations are recorded as unexcused absences. In the event a family has a planned vacation, a parent/guardian is responsible for submitting a Planned Absence Form. Submission of this form does not excuse a student’s absence.
This form will serve as notification to a student’s school administration of a family's planned vacation and must be submitted no less than two weeks prior to the student’s absence.
