• Registration Checklist

     

    •  I have completed the online registration form and have received confirmation from a member of the District 65 registration team.
    •  I have submitted a copy of my student's birth certificate (the original has been verified in person by a District 65 registration team member).
    •  I have submitted ALL residency verification documents (proof of address) to the District 65 registration team. For Spanish-speaking families, the Spanish version of the residency verification form has also been provided. 
    •  I have submitted a copy of my student’s recent health documentation to the District 65 registration team.
    •  I have completed all required transfer documents (only for students transferring from another Illinois public school).
    •  I have submitted any applicable documents, such as court records, IEP/504 information, and other essential school related documentation to the District 65 registration team.
    •  I have paid all of my student fees.
