    State of Illinois law requires school districts to have certain health information on file for students entering a specific grade including verification of immunizations as well as physical, dental, and eye examinations. 
     
    immunizations 
    Immunization dates and records must be submitted on the Child Health Form, signed, stamped, and dated by a health care provider. In addition, a complete list of immunization requirements is available in six different languages at http://www.immunize.org.
     
    Please note that beginning in the 2013-2014 school year, the Illinois Department of Public Health began requiring all students entering, advancing, or transferring into sixth through eighth grade to show proof of receipt of a dose of Tdap vaccine (combined tetanus, diphtheria, acellular pertussis) regardless of the interval since the last DTaP, DT, or Td dose.
     
    physical examinations 
    State of Illinois law requires proof of a physical examination dated no earlier than one year prior to a student entering kindergarten and sixth grade, for all students entering the district for the first time, and for all students entering District 65 from outside the State of Illinois or the United States.

    dental examinations 
    In compliance with Illinois Public Act 093-0946, by May 15 of that school year, students entering kindergarten, second, and sixth grade are required to have a dental exam on file. School dental examinations must have been completed within 18 months prior to the May 15 deadline.

    eye examinations 
    State of Illinois law requires proof of a comprehensive eye examination for all children entering kindergarten, or enrolling in a public, private or parochial school in Illinois for the first time. The examination must have been completed within one year prior to the child beginning school and submitted to the school no later than October 15 of the year the child is first enrolled.

    vision and hearing screenings 
    Vision and hearing screenings are completed annually for all students in pre-school, special education, kindergarten, second and eighth grade, and for all students new to the district. In addition, vision screenings are completed for students in fifth grade. Hearing screenings are also completed for students in first and third grades. Vision or hearing screenings may also be requested by a parent or teacher.
     
    kindergarten health requirements 
    Before your child can attend the first day of kindergarten, the following health requirements must be submitted to the Health Services Department: 
    • A completed physical examination form (exam must be completed within one year prior to the first day of school) including:
      • a required blood lead test or assessment in compliance with Section 7.1 of the Lead Poisoning Prevention Act
      • all applicable immunizations
    • Proof of a comprehensive eye exam is due by October 15
    • Proof of a dental exam is due by May 15 of the current school year
    Note: Students entering District 65 School Age Child Care require proof of a TB test completed within one year prior to the first day of childcare
     
    Evanston Township High School Health Requirements
    Before registering for high school, a completed physical exam dated within one year of the high school start date is required. This also applies for students who are attending ETHS summer school.
