Before a child can enroll in school, Illinois law requires proof of the following:

Physical Exams
A physical examination dated no earlier than one year prior to entrance for students entering kindergarten and sixth grade and for all students entering our district from out-of-state or out-of-country. The form is available on the Illinois Department of Public Health or can be printed from the Forms tab on the right.

Immunization Requirements
Illinois state law requires immunization records to be on file.  A schedule of Child and Adolescent Immunizations is available on the Centers for Disease Control website.

Immunizations must be verified in writing by a doctor. All immunization dates (MM/DD/YYYY) must be filled in and the health care provider must sign the immunization record.  If proper immunization documentation cannot be found, the child should be re-immunized.  A printable copy of the form also is available on this page (see forms on the right).  Information about when children and teens need vaccinations is available in six languages at http://www.immunize.org/

Dental Exams
In compliance with Illinois Public Act 093-0946, by May of that school year, students entering kindergarten, 2nd and 6th grade are required to have a dental exam on file.  The dental exam form is available on the Illinois Department of Public Health website or can be printed from the Forms tab on the right.

Eye Exams
Illinois law requires proof of an eye examination by an optometrist or physician who provides complete eye examinations to be submitted to school no later than October 15th of the year the child is first enrolled or as required by the school for other children. The examination must have been completed within one year prior to the child beginning school.  The eye exam form is available on the Illinois Department of Public Health website or can be printed from the Forms tab on the right.

Vision and Hearing Screenings are done annually for all students in pre-school, special education, kindergarten, second and eighth grade as well as for all new-to-district students.  Vision Screening also is done for students in fifth grade.  Hearing Screening also is done for students in first and third grades.  Additionally, vision or hearing screenings can be requested by a parent or teacher.

Kindergarten Health Requirements
All Illinois health requirements must be met prior to registering a child for kindergarten.  This includes the physical examination done within one year prior to the first day of school (physical exam form can be printed from the Forms tab on the right), a required blood lead test or assessment in compliance with Section 7.1 of the Lead Poisoning Prevention Act, all necessary immunizations, evidence of a dental examination, and, for students entering District 65 School-Age Child Care, results of a TB test completed within one year prior to the first day of child care.

Evanston Township High School Health Requirements
Before registering for high school, a completed physical exam dated within one year of the high school start date is required.  This also applies for students who are attending ETHS summer school.  Please visit the Evanston Township High School web site for more information.
